2013-2014 ENC Medical Plans Summary of Benefits & Coverage ("SBC")
As required under the Patient Protection and Affordable Care Act, group health plans are required to provide an easy-to-understand summary about a health plan’s benefits and coverage.
You will find the Summary of Benefits & Coverage ("SBCs") for each of ENC’s three medical plans for fiscal year 2014 (July 1, 2013 – June 30, 2014) below. The SBCs summarize the key features of each plan, such as the covered benefits, cost-sharing provisions, coverage limitations and exceptions in short, plain language to help you compare health plans.
While every effort has been made to make this description as complete and accurate as possible, the SBC cannot contain all terms and provisions of the Plan. If any conflict should arise between this summary and the Plan or the contracts, the terms of the Plan and insurance contracts will govern in all cases.
You may download and print the electronic versions of these documents at no charge by using your office's printer. Should you wish to receive a paper version of the SBC documents, you may pick up a copy in the Office of Human Resources, or you can request the Office of Human Resources and such documents will be mailed to you free of charge. You are responsible to share the document with your family members or others who are covered with your insurance.
The ENC Health Care Program is generally available to most full-time employees who are regularly scheduled to work 30 hours or more a week. This information does not include all of the details contained in the applicable insurance contracts, plan documents, and trust agreements. If there is any discrepancy between this information and the governing documents, the governing documents will control. ENC expressly reserves the right to change, amend or cease its benefits and plans at any time, for any reason or no reason whatsoever.
